Marriage License Forms
Marriage License Application
This is the application to be filled out for a marriage license. You can print this form from here and bring it into our office along with both parties' birth certificates to save on time.
Covenant Marriage Packet for Soon-to-be-Married Couples
This packet must be filled out and returned to the Clerk of Court office at the time the couple wishes to obtain their Marriage License. These forms are intended for couples who are not yet married, but wish to have a Covenant Marriage.
Covenant Marriage Packet for Already Married Couples
These forms may be filled out and submitted to the Clerk of Court office. They are intended for couples who are already married and wish to convert their marriage to a Covenant Marriage.

Request for Cancellation Form
This form must accompany all requests to cancel a lien, whether they be by original paid note or order to cancel.
Release by Obligee of Record Form
This form may be used by the Obligee of a lien, accompanied by the Request to Cancel, to release a lien from record.
Request to Cancel by Licensed Financial Institution
This form may be used, without an original paid note, by a Licensed Financial institution to cancel a mortgage or privilege from record whenever the signature of one loan officer is utilized, along with a notary for that signature.
Req for Cancellation by Licensed Financial Institutional (one LFI officer and a notary OR two LFI officers without a notary)
This form may be used, without an original paid note, by a Licensed Financial institution to cancel a mortgage or privilege from record using one of two signature methods. Whenever the signature of one LFI officer is utilized, the signature of that officer must be notarized. A notary is NOT required on this form if TWO officers sign the request. See Page 2 of this form for signatures.
Request to Cancel by Prescription
This form may be used to cancel a mortgage if the request is based on a Mortgage or Small Claims Judgment's prescription.
Request for Partial Release
This form may be used by any non-bank party to request a partial release from a lien. It must be accompanied by an authentic act of partial release.
Request for Partial Release by LFI
This form may be used by the lending institution that is the current holder of a lien and wishes to release a portion of said lien.
Request by Obligee of Record (Lost Note)
This form may be used by the obligee of record of a mortgage or vendor's privilege which was secured with a paraphed promissory note that has since been satisfied and lost (or destroyed).
Discharge Record Request Form
This form may be filled out and either mailed into our office or brought into our office to request a copy of a Discharge Record. The requirements and and specifications for those who are allowed to request Discharge Records are the same as those for Birth Certificates.
