Forms for passports are available on our website or online at travel.state.gov OR the Jackson Parish Clerk of Court office.

Form must be filled out in BLACK INK.

YOU MUST SIGN THE FORM IN OUR PRESENCE.

You will need the following:

  1. 1-2x2 passport photo (available at Jonesboro Wal-Mart)
  2. Certified copy of Birth Certificate OR expired passport if issued after you were 16 years of age and less than 15 years ago
  3. Drivers License
  4. Money order or check made out to the U. S. Department of State in the amount of:
    $130 passport for adult* (16 years or older)
    $100 if applicant is under 16 years of age* (both parents must be present to sign & have drivers license)
  5. $35 separate fee to Jackson Parish Clerk of Court (cash, money order, or check)

If you need to expedite make money order or check out for:
$209.53 adults ($19.53 OF THIS FEE IS OPTIONAL. YOU MAY DECLINE RETURN OVERNIGHT SERVICE.)
$179.53 applicants under 16 ($19.53 OF THIS FEE IS OPTIONAL. YOU MAY DECLINE RETURN OVERNIGHT SERVICE.)
This includes application fee, $60 expedite fee & $19.53 overnight return fee.
You may purchase an overnight envelope from the post office and bring to us to send the application.

Passport card (not valid for travel by air) (CANNOT BE RETURNED BY OVERNIGHT SERVICE)
